I (the undersigned) understand and agree to the following information:

• The Ammortal Chamber uses pulsing electromagnetic energy and that this energy can disrupt pacemakers, defibrillators, insulin pumps, implanted cochlear hearing devices and other implants that are battery operated or magnetically charged in or on my body. I attest that I do not have any such devices in my body and will remove any such devices that are on my body before entering the Ammortal Chamber.

• I have not had a joint injury within the past 48 hours

• I do not have chronically hot or swollen joints

• I do not have an enclosed infection (dental, in joints or other tissue)

• I do not have hemophilia, actively bleeding or am predisposed to hemorrhage

• I do not have seizures or blood clots

• I have not received chemotherapy in the last 48 hours

• I do not have a fever or a condition that makes me sensitive to heat or any condition associated with impaired

  sweating

• I am not under the influence of drugs or alcohol

• I am not pregnant

• I understand that the Ammortal Chamber is not intended to take the place of medical care or medications. I

  understand that I take full responsibility for my own health and well-being and acknowledge that the results of the

  Ammortal Chamber will vary and that no guarantees of specific results are offered or implied.

• I agree to wearing the supplied protective goggles throughout my entire session in the Ammortal Chamber

• I will ensure that no other person, child, or pet will be able to touch me or the unit during my session

• I will not use any electrical device during my Ammortal chamber session

• I agree to keep my arms and limbs within the confines of the bed during my entire session

   

• The Consenter agrees to hold Ammortal Inc (“Releasee”) harmless of all legal, financial, and any other liability that includes their agents, employees, successors and assigns, and their respective heirs, personal representatives, affiliates, successors and assigns, and any and all persons, firms or corporations liable or who might be claimed to be liable, whether or not herein named, none of whom admit any liability to the undersigned, but all expressly denying liability, from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, which have or may hereafter have, arising out of or in any way relating to any and all injuries and damages of any and every kind, to both person and property, and also any and all injuries and damages that may develop in the future, as a result of or in any way relating to the Consenting Acts.

• It is understood and agreed that this Agreement is made and received in full and complete settlement and satisfaction the causes of action, claims and demands mentioned herein; that this Consent contains the entire Agreement between the parties; and that the terms of this Agreement are contractual and not merely a recital.

• Furthermore, this Consent shall be binding upon the undersigned, and his respective heirs, executors, administrators, personal representatives, successors and assigns.

  This Consent has been read and fully understood by the undersigned and has been explained to me.
